Immunizations:
State law requires that all students be currently immunized against certain communicable diseases to be enrolled in school. The following is a list of required immunizations for high school students in the State of Texas.
Diphtheria/Tetanus: At least 3 doses, with one being within the last 10 years
Polio: 3 doses with one being after the fourth birthday.
Measles, Mumps, Rubella: Two doses after the first birthday.
Hepatitis B: 3 doses
Chickenpox (Varicella): 1 dose after the first birthday if immunized before age 13. If immunization given after age 13, 2 doses are required. If student has had chickenpox disease, parent may submit a form confirming the disease with the date that the student had the disease. Forms may be obtained in the school nurse’s office.

Medications:
All prescription medications must be brought to the school nurse by a parent. Prescriptions must be current and be in the original container with a correct prescription label. Parents must also send a note giving permission for the medication to be administered. The note should include the student’s name, name of medication, dose and time(s) to be given. Medication forms may be picked up from the school clinic. The nurse may at times also require a doctor’s note. Neither the nurse nor any school employee may administer herbal or dietary supplements. Students may carry non-prescription medications if they are in the original container and brought to the clinic to be taken. Parents may bring over the counter medications to the nurse to be given on a short term basis. These medications must be in their original containers and be accompanied by a parent note. If a student must receive an over the counter medication for longer than 2 weeks, a doctor’s note must be received.
