Medication Policy |
All medication must be in the original container, properly labeled with the student’s name, name of the medication & dosage.
A parent or guardian must pick up and deliver all prescription medications. A Medication Authorization form must be signed by the doctor & returned to the clinic within 1 week of beginning a medication or whenever a medication dosage is changed.
Non-prescription medications (Tylenol, Advil, Lactaid, etc.) may be given with written parental permission for 2 weeks. Only the dosage listed on the package insert will be given. Medications required longer than two weeks must be accompanied by a Medication Authorization form signed by the student’s physician.
Students may not share prescription medications with siblings.
No vitamins, herbal medications or health foods will be administered by the clinic.
Please note that due to the danger of Reye’s Syndrome, no aspirin or aspirin-containing product will be administered by the clinic staff unless expressly ordered by your child’s physician.

Immunizations |
Texas state law requires all students to have up to date immunizations to be admitted or remain in school.
Diphtheria/ Tetanus - 3 doses are required with at least one dose on or after the 4th birthday and a booster every 10 years thereafter. |

Polio |
3 doses with at least one dose on or after the 4th birthday. |

|
|
 |
Measles |
2 doses of the vaccine for students born on or after 9/21/91. For students born on or after 9/1/78, two doses are required, one on or after the first birthday, the second dose at least 30 days later but at least by the 12th birthday.
