Checklist of Registration for Klein ISD
Children entering kindergarten and first grade for the first time who were not pre-registered in the spring will need:
  • Official birth certificate (hospital certificate not accepted)
  • Complete immunization health record. See immunization requirements below.
  • Completed registration card for attendance and emergency procedure card to be used in the event of an accident, etc. (form supplied by the school)
  • Records from kindergarten, if child attended (not mandatory, but will be helpful in grouping)
  • Proof of residence in the Klein ISD. A lease agreement, light, water, or gas bill with your name and address is acceptable. Telephone bills cannot be accepted.


Students transferring from other districts, grades 2-5, will need:
  • Proof of identity
  • Transcript of work (report card, progress report, evaluation report that is printed, written and/or signed by school personnel to designate grade level). It is the responsibility of the parents to secure this information.
  • A statement, book card, withdrawal form, or computer card clearing their record from the previous school.
  • Immunization record, either from the previous school or the family doctor.


The following immunizations are required for enrollment in the Klein schools:
  • HIB: Students under the age of 5 are required to have 1 dose of HIB at 15 months or older unless a series Hs been completed with a booster when the child was less than 15 months.
  • DPT/DtaP: Students under 7 years of age are required to have 4 DPT/DtaP with one being after the 4th birthday. It is acceptable if the dose is in the calendar month prior of the fourth birthday.
  • Diphtherial/Tetnus (Td): For students 7 years and older - 3 doses required with at least one dose after the fourth birthday and within the last ten years. It is acceptable if the does is in the calendar month prior to the fourth birthday.
  • Polio: At least three doses of either oral or injectable polio with one dose after the fourth birthday. It is acceptable if the does is in the calendar month prior to the fourth birthday.
  • Measles, Mumps, Rubella: One dose on or after the first birthday. At age 12, students are required to show proof of a second measles vaccination. Two doses of measles are required for students 5 years and older who were born on or after September 2, 1991. If a student has prior illness of measles, mumps, or rubella, serologic conformation is required in lieu of vaccination.
  • Hepatitis B: Children born on or after September 2, 1992, must have three doses of Hepatitis B vaccine by age 5. Students born between September 2, 1988 and September 1, 1992 (inclusive) are required to show proof of three doses of Hepatitis B vaccine by their 12th birthday.
  • Varicella (chickenpox): Children born on or after September 2, 1994, must have one dose of Varicella vaccine on or after first birthday or documentation of Varicella disease. Students born between September 2, 1988, and September 1, 1994 (inclusive) must have one dose of Varicella by their 12th birthday.

    Exclusions from compliance:
    Exclusions from compliance are allowable on an individual basis for medical contraindications and religious conflicts. Students in these categories must submit evidence for exclusion from compliance as specified in Texas Education Code, Chapter 38 and the Human Resource Code, Chapter 42.

    Medical exemptions:
    The student must present an affidavit or certificate signed by a physician, duly registered and licensed to practice medicine in the United States, in which it is stated that, in the physician's opinion, the immunization required would be injurious to the health and well-being of the applicant or any member of his or her family or household. Unless a lifelong condition is specified, the affidavit is only valid for one year.

    Religious conflicts:
    A signed affidavit must be presented by the student's parents or guardian stating that the immunization conflicts with the tenets and practices of a recognized religious organization of which the applicant is an adherent member.

    Exemptions do not apply in times of emergency or outbreak declared by the commissioner of health or local health authority.
