Parent Information
The clinic uses only heat, cold, Vaseline, calamine, hand lotion, disinfectant soap, salt water and hydrogen peroxide for the care of our students. We do not have any medication in the clinic unless it is brought from home. An adult must bring and pick up any medications that are given to the clinic. A permission note from the students guardian must accompany the medication. The date, dose and time for medication to be given should be included in the permission slip.

· The medication must be kept in the school clinic
· The medication must be in its original container.
· It must be age appropriate.
· Prescription medicine must be correctly labels for that student.
· Inhalers must be labeled with the students name and directions for usage.
· Medication prescribed “three times a day” can be given before and after school and before bed.
· Over the counter medication can be kept in the clinic for two weeks without a doctor’s note.

PE

Parent/Guardians may write a PE/recess restriction for one week. A Dr.’s note is necessary for a longer excuse. The doctor’s office will need to specify when PE activity can resume. The excuse is brought to the clinic and copies are made for the teacher and coach. A P.E. excuse sticker is worn by the student the first day of the note.

Updates

· Inform the school nurse of any medical concerns that your child experiences (asthma, ADD, allergies, disabilities, seizures, surgeries, etc.) A care plan may be composed to help others know how to aid your child with symptoms.
· A change of clothes should be kept in all Pre-K, KN, EC students backpack.
· Current phone must be updated immediately. The nurse always needs a way to reach you.

Clinic Pass

Each time your child comes to the clinic their teacher sends them with a pink pass. The teacher will send your students passes home in their folder. If you have any questions about the clinic visits, please feel free to call us.

Clinic Screenings

* Vision and hearing: For KN, 1st, 3rd and 5th grade. Scheduled in September.
Each child’s results are sent home on a pink slip. Retests are done on those that
those that do not pass.

* AN/Acanthosis Nirgricans: For 3rd & 5th grade. Students are observed for a
marker on their neck. This can appear if the body has had prolonged elevated
levels of insulin. This can be a warning sign for Type II diabetes in later years.

* Scoliosis: For 5th grade. This is held in January. The nurse will call you if any
Symptoms are observed.

Prevention

· Keep your child’s immunizations up to date. Always bring your child’s shot
record with you when you go to update your shots. Harris County Health Clinics
offer immunizations for $15.00/sliding scale. Texas state law requires current
records in order to attend classes.

· The best way to keep your child healthy is to have them wash their hands with
friction and soap for about thirty seconds or sing Happy Birthday to yourself.
Before they eat, after using the restroom and cough/sneeze.

* When you call attendance to report your child’s absence, please let them know
your child’s symptoms. The clinic keeps record of the contagious diseases in
each class. Your student may not attend class unless they are fever free (without
tylenol or ibuprofen) for 24 hours. Children must be diarrhea (without
medication) free.
