Klein ISD Immunization Requirements
2006-2007

Diphtheria/Pertussis/Tetanus (DPT)
Minimum 4 doses. Last dose must be administered no earlier than 4 days before the 4th birthday but can be administered any time after that.

Polio (OPV or IPV)
Minimum 3 doses. Last dose must be administered no earlier than 4 days before the 4th birthday but can be administered any time after that.

Measles, Mumps, Rubella (MMR)
Minimum 2 doses. First dose must be administered no earlier than 4 days before the 1st birthday. Second dose must be given at least 30 days after the first; can be just Measles. Often given with 4 yr. shots.

Hepatitis B (HepB)
3 doses. First dose at birth or soon after, second dose at least 4 weeks later, third dose at least 8 weeks after second dose.

Varicella (Chicken Pox)
1 dose, no earlier than 4 days before the 1st birthday. If student has had chicken pox, must have note by parent or guardian stating the approximate date of the disease.

Haemophilus Influenza type B (HIB)
ONLY REQUIRED FOR STUDENTS UNDER AGE 5. One dose, if given after 15 months. Otherwise, a series of 3 doses, including a booster after age 1.

Hepatitis A (Hep A)
ONLY REQUIRED FOR PRE-K
2 doses, 6 months apart

Prevnar (PCV)
ONLY REQUIRED FOR PRE-K
Series of 3 doses with one after 1st birthday, or only one dose if given after 24 months.
