IMMUNIZATIONS******************************************************

Texas State law requires that all 7th graders have the following immunizations before they are allowed to begin the first day of school:

  1. TdaP(Tetanus Diptheria, Pertussis),
  2. Meningococcal(Meningitis)
  3. Varicella( a second chicken pox)

    The Doerre Clinic MUST have a copy of these shots BEFORE the first day of school. These records may be e-mailed to schilton2@kleinisd.net any time over the summer- please confirm that they were received and reviewed.  They may be dropped off in the front office or faxed to the clinic after August 15th.                       

Screenings:

ALL new students to Klein ISD and all 7th graders are screened for the following during the school year.

*Vision and hearing - Thank you for following up with a specialist and returning the results  to the clinic . The return of this report to the school is required by the state if follow up is to be complete. If you cannot afford vision care for your child, please contact the clinic, as we have many free and reduced cost resources.  If your child is reporting having trouble seeing or hearing in class and you would like to have your child in ANY grade screened, please contact us.        Acanthosis Nigricans(AN) is a skin marker found on the back of the neck that we also look for during our 7th grade screenings. It is a possible result of high insulin levels in the blood , which can be a precursor toType II Diabetes(which is on the rise in teenagers in the U.S.) If we feel that the student has this marker, we will also check his/her blood pressure and will notify you within the next couple of months. Further information about AN will be mailed to you along with the referral letter to take to your child’s physician. In Texas, early detection and disease prevention are promoted through
measures such as this.


*Growth Assessment- height and weight were obtained on your child in a private setting. Please contact the clinic if you need any information regarding this.

8th grade screenings:

*Spinal screenings are done on all of our 8th graders .  If abnormalities were detected,  a referral letters was mailed to your home regarding the need to follow up with your child’s physician . 


Since routine screenings are not done in high school, we hope that you will follow up with any potential health problems that you are notified about concerning your child during these middle school years. If you need any information regarding financial resources available for health care, please let us know- we would like to help!

Allergy Action Plan Forms
If your child has a life threatening food , insect , or environmental allergy, please contact the clinic so that we, along with your child's physician, can develop a plan to keep your child safe and healthy at school. You may download these forms to take to the Dr and return them to the school nurse , along with any medication that is needed.

Medication Authorization Form
A)If your child has a need for medication at school, it must be given in the clinic. If it is an over the counter medication(non prescription), it may be brought to the clinic by your student, along with a note from you, stating the:
1.name of the medication
2.name of the student
3.dosage to be given
4.time to be given
The medication must be in the original container and not be expired. Dosages must be given only as directed on the label, according to your child's AGE(not weight) , per district policy. No herbal medications or supplements may be given by the school nurse- only FDA approved medications. Over the counter medications can be administered by /kept in the clinic for up to two weeks without a Dr order, but if they are to be given at school after that time, a Dr note is required.

B) Prescription medications MUST BE BROUGHT IN BY THE PARENT or another designated adult. They must include the prescription label and must be given as stated(dosage, time, and amount) on the label. These include ADHD meds, inhalers, epi pens, antibiotics, and other prescription medications. Narcotics may not be administered at school. Permission to carry asthma inhaler
If your child's physician has determined that it is medically necessary for your child to carry his/her asthma inhaler while at school for quick access, the attached form must be completed by the Dr and the inhaler must be brought in by the parent the first time it is brought to school. The student will be provided with a copy of this form which must be kept with the inhaler, in his/her possession at all times. Please contact the clinic for any questions. If your student is in athletics at Doerre and you feel that they may need access to their inhaler during after school hours, please contact the clinic and your student's coach.
