Please contact the School Nurse to discuss your child's seizure disorder. 
  • Students with a history of seizures should submit an annual Seizure Action Plan signed by the physician and parent. This will allow the School Nurse to monitor and manage the student’s seizure activity in accordance with your physician’s orders.

  •  Seizures can be unpredictable and it is hard to tell how long they will last or what will happen during them. Some may start with minor symptoms, and then lead to a fall or a loss of consciousness; other seizures may end in just a few seconds 

  •  Student’s who require medication at school to help control seizure activity must submit the Klein ISD Medication Authorization Form in addition to the Seizure Action Plan to the School Nurse.
     
  • This form can be obtained from the school nurse or located in the Seizure Packet below. 
  • If your child requires Diazepam Rectal Gel as an emergency intervention to control or stop a seizure the Diazepam Rectal Gel (Diastat) Orders must be completed in addition to the Seizure Action Plan by both the parent and physician.
